Pre-Practicum Preceptor/Mentor, Faculty, and Learner Conference

A preconference will occur prior to the start of the practicum/clinical experience. This meeting is intended for the learner and preceptor/mentor to review course- and learner-specific learning objectives: the roles, responsibilities, and expectations of learner and preceptor/mentor during this clinical experience. All faculty and preceptor/mentor contact information will be given to each party per the learner. Any course or program information requested by the preceptor/mentor will be provided by the learner. During this meeting, the learner and preceptor/mentor will discuss practicum/clinical/practice goals, including any projects that are to take place. Faculty will review submitted document and need to approve proposed projects at this time. Faculty will also send an introductory e-mail to the preceptor/mentor.

In order to receive a passing grade in the course, all course evaluations must be completed by preceptors/mentors and submitted by learners.

Learners and preceptors/mentors will attest to meeting by signing the “Preceptor-Faculty-Learner Conferences-Evaluations” form and submitting it to the instructor using the assignment drop-box during the first week of class for approval of the proposed experiences.

A mid-term conference will occur at the mid-point of the course. This meeting is intended for the learner, faculty, and preceptor/mentor to discuss learner progress toward meeting the course goals.  The faculty member will meet with the preceptor/mentor to conduct the learner’s midterm evaluation. The preceptor/mentor will meet with the learner to review the evaluation form. If there are areas that need improvement, the faculty will meet with the learner to complete a remediation form.

A final clinical evaluation will occur at the end of the clinical/practicum experience. This is intended for the learner and preceptor/mentor to review and evaluate all competencies and validate that all areas are at “meets expectations” prior to progression. Faculty will provide final review and evaluation after conferring with preceptor/mentor.
