Purpose
The purpose of this assignment is for the student to describe exactly what EBP change will be implemented as part of their proposed DNP project.

Course Outcomes
This assignment provides documentation of student’s ability to meet all project and practicum course outcomes and fulfill practicum hour requirements.

Preparing the Assignment
Provide full elaboration here to explain exactly what EBP change you will implement as part of your proposed DNP project. Do not leave any questions left unanswered and provide your reader with key details so they will understand the intervention and supporting evidence. Include a minimum of 3 scholarly citations to support the intervention in this section. Remember you are not conducting original research but are implementing an intervention supported by research to change practice and patient outcomes. The evidence-based intervention should not be modified as this is a form of research.

The following are the requirements for the evidence-based intervention section:
Rationale for chosen evidence-based intervention is provided and up-to-date references support the intervention.
Discusses how chosen evidence-based intervention is appropriate for project site and
Resources.
Discuss chosen evidence-based intervention in-depth including each of the following aspects:
Protocol illustrated as a Table or Appendix (as applicable)
Information Sheet illustrated as a Table or Appendix
Informed or Implied Consent process discussed and illustrated as an Appendix
Education plan discussed and illustrated as a Table or Appendix
Plan for implementation identified and discussed. The reader should be able to implement your intervention without any questions left unanswered.

Organized around the required components using appropriate headers.
